Warri Wolves responded swiftly. Substitute Peter Ita made an immediate impact with a superb run before delivering a precise cross for Emmanuel Arigbogha, who powered in a diving header at the far post in the 73rd minute to restore parity.
Both sides pushed for a winner in an open finish to the game, but missed chances meant the encounter was decided from the penalty spot.
De Sapele Lions held their nerve, converting three of four attempts, while Warri Wolves missed three of their five kicks.
